I think the standard is waste pipe down into the floor, with a water tap behind it mounted on the wall, although that is less of a problem.

There are three different type of waste traps. An ‘S’ trap is the type you’re talking about, where the plumbing is connected through the floor. A ‘P’ trap has the plumbing connected through the wall behind the toilet. A ‘Skew’ trap – which sounds like Aeek is talking about, has the trap coming out the side, to connect to the plumbing in a side wall. I’ve never seen one of these in a toilet, but Caroma still make a model (just the one though!). Please don’t repeat our mistake of assuming that a plumber would not be stupid enough replace a high-flush cistern with a low-flush cistern, leaving a high-flush bowl that no longer had enough water supply to flush properly.

I recently had close coupled toilets fitted with a fantastic flushing action. There are the usual two flush buttons and usually both number ones and number twos are adequately managed by the lesser flush but if a bit more is required all that has to be done is the low flush button to be held down.
I have never had any reason to use the high flush. The set up of the drain holes under the bowl rim is a lot different to the standard Fowler one and this contributes to the efficiency. These designs save a lot of water.
